The training or weight control center must provide you with a contract which details the conditions of your membership. By law, this is a "health studio contract". Make sure this contract reflects the promises you made to the merchant.
The contract must indicate:
the license number issued by the Consumer Protection Office;
the name and address of the merchant;
your name and address;
the place and date of the signing of the contract;
description of the services offered;
the date on which the services will begin to be offered;
the address to which the services will be offered;
the duration of the contract (maximum one year);
the total amount you have to pay;
the statement explaining the terms of cancellation of the contract. This statement is presented in Article 47 of the Implementing Regulation of the Consumer Protection Act.
The center must give you a duplicate of the signed contract, along with a termination form. You can use this form if you want to cancel your contract.
TOTAL AMOUNT OF THE CURRENT CONTRACT
The contract must indicate the total amount you will have to pay. This amount includes:
the cost of the membership or session;
membership, registration, file opening, evaluation, and administration fees, if applicable;
the taxes.
